When it comes to your healthcare journey, your personal goals play a pivotal role in achieving success. Sharing your personal goals with your healthcare team creates a collaborative atmosphere. This means that your treatment plans and strategies are not only based on medical expertise, but are also tailored to align with your aspirations, lifestyle, values, and priorities. Establishing clear personal goals encourages open and honest communication between you and your healthcare providers and this transparency builds trust and fosters a stronger relationship.
Follow these steps to add your personal goals to an Action Plan:
- Click on the “Action Plan” tab.
- Scroll down to the Personal goals section and click on “+ Add”.
- Enter in a title for your personal goal (i.e. Take a flight, walk, graduation, etc).
- Write a short description of the goal to provide some context for you and others. (i.e to be able to walk around the block).
- Upload a photo for motivation if you choose.
- Click “Add Goal”.
To edit or mark the goal off as complete, follow these steps:
- Click on the “Action Plan” tab.
- Scroll down to the Personal goals section and click on the edit button of the goal in question.
- Click “Edit” to make changes and click “Save” when you are done.
- Click “Complete” to indicate the goal has been achieved.
