Personal goals are key to your healthcare journey. By adding your goals to your Action Plan, you’re creating a roadmap that supports your progress and helps your care team understand what’s important to you.

Why this matters
Goals are a key part of person-centered care. They help your team understand what you’re working toward and how to best support you. Plus, tracking goals can help you celebrate progress and stay motivated.
How to add and track goals
1. Add a new goal
Go to the About You section in your Action Plan.

Click + Add to enter a new goal (e.g., "Walk to the mailbox and back").

Click Save to add it to your plan.
2. Mark a goal as complete
When you achieve a goal, click Complete. Your progress will be updated automatically.
3. Hide completed goals
Once you complete a goal, you can choose to hide it to keep your focus on active goals.
4. Edit or delete goals
To update or remove a goal, click the three dots (…) next to the goal, then select Edit or Delete.
How to review your goals
Go to your Action Plan and scroll to the Goals section. You can see your progress, completed goals, and any goals added by your care team.
