Why this matters

Sometimes Action Plans are archived when care is completed or paused. But if you or your support team need to see the information again, it’s possible to reopen the Action Plan—just reach out to the team who first created it.


How it works

Careteam doesn’t control access to Action Plans—your healthcare team does. That’s why the first step is always to connect with the healthcare provider or organization that originally created your Action Plan.


Steps to request access

  1. Find the right contact
    Think back to which healthcare provider or organization first set up your Action Plan.

  2. Reach out to them directly
    You can contact them by phone, email, or whichever method you’ve used before.

  3. Ask to reactivate your Action Plan
    Let them know you'd like access to your previously archived Careteam Action Plan.


Cool things to know

  • Only the healthcare team who created the Action Plan can reactivate it.

  • Once reactivated, you’ll be able to see all your past tasks, appointments, notes, and resources again.

  • If you're not sure who to contact, reach out to any provider on your care team—they may be able to help direct you.
