📝 What to include

  • What happened – Describe what you were doing and what went wrong

  • Where it happened – Let us know what part of Careteam it was (e.g., “Action Plan”, “Check-in”, “Profile”)

  • Screenshots or screen recordings – Optional but super helpful!
