Why this matters

Adding health details to your Action Plan ensures your care team and supporters always have the most up-to-date information. It helps everyone—from clinicians to family—make informed decisions and support your health journey effectively.


How to add health information

Go to your Action Plan and open the Information section in the person's panel.

Click + Add to choose from the following categories:

🔹 Current Medications
List the medications you’re currently taking (e.g., “Metformin 500 mg – twice daily”).

🔹 Past Medications
Record medications you’ve stopped and why, to avoid confusion or duplication.

🔹 Immunizations
Keep a record of vaccines you’ve received and when (e.g., COVID, flu, tetanus).

🔹 Allergies
Add any allergies (e.g., penicillin, peanuts) so everyone is aware of potential risks.

🔹 Family History
Note any relevant family health history (e.g., heart disease, diabetes).

🔹 Notes
Use this space for general updates or summaries from appointments and discussions.

Click Save to store each item. You can always edit or delete entries as things change.
