After your health care provider has created your Action Plan, you will receive an invitation with a sign up link to your email inbox (if you don’t see it, check your spam folder for an email from 

To activate your Action Plan follow these steps:

  1. Click the “Join Now” button.
  2. Create your profile. 
    • Enter your name.
    • Create a password.
    • Upload a profile picture (you can also do this later).
    • Read and agree to the Terms of Service. 
  3. Accept the invitation to your Action Plan.
