Sometimes Action Plans are archived when care is completed or paused. But if you or your support team need to see the information again, itâs possible to reopen the Action Planâjust reach out to the team who first created it.
Careteam doesnât control access to Action Plansâyour healthcare team does. Thatâs why the first step is always to connect with the healthcare provider or organization that originally created your Action Plan.
Find the right contact
Think back to which healthcare provider or organization first set up your Action Plan.
Reach out to them directly
You can contact them by phone, email, or whichever method youâve used before.
Ask to reactivate your Action Plan
Let them know you'd like access to your previously archived Careteam Action Plan.
Only the healthcare team who created the Action Plan can reactivate it.
Once reactivated, youâll be able to see all your past tasks, appointments, notes, and resources again.
