A quick guide to accepting your invitation and getting started on Careteam Your healthcare team is using Careteam to help you stay organized, informed, and supported. This platform brings together your care providers, family, and support systemâall in one place. Whether the Action Plan is for you or someone youâre supporting, this article will walk you through how to sign up and get started.

There may be a lot to keep track ofâappointments, resources, tasks, and updates from your care providers. Your Action Plan helps make it easier by giving you the right information at the right time, in one place.
Careteam is designed to support people receiving care and their families and care teams, so everyone can stay connected.
If youâve been invited to Careteam, youâll receive an email from support@careteam.me. It will include:
The name of the person inviting you
The organizationâs logo
A button that says Access now
If you donât see the email, check your spam or junk folder.
Click Access now in the invitation email
A new window will open where youâll:
Add your first and last name
Create and confirm a password
Click Next
Youâll see the name of the Action Plan you were invited to (e.g., Recovery Plan)
Click Accept invitation
Thatâs it! Youâll be brought straight into the Action Plan.
Once youâre inside, you can explore your Action Plan:
Review any tasks or resources
Send and receive messages (if messaging is enabled)
See who else is on the care team
Add or update your own contact details and preferences
You can also bookmark the Careteam site or save it to your phoneâs home screen so itâs easy to return to.
You only need to sign up onceâafter that, just log in anytime at careteam.me/login
Youâll get email notifications when something new is added
If you're supporting someone else, youâll still see everything relevant to their care
You can join more than one Action Plan using the same login (for example, for multiple family members)
