As part of your care plan, a health care provider may have created tasks to support your success. An Action Plan task can be anything from reading a document, to completing daily exercises, or scheduling an appointment. Tasks help you know what to do as assigned by your health team and inform your health team and primary supporters when that task is complete.

Follow these steps to review your tasks and mark them off as complete:

  1. Navigate to the Tasks section. Note: when you sign into Careteam it will bring you directly to the Task page. 
  2. Click on the title of the Task to review more details. You can access the resource link (if one is added), claim responsibility for completing the task, edit the task, or mark the task off as complete
  3. View completed tasks by clicking on Show completed.
