You may be no stranger to filling out forms- intake, self-assessments, surveys and administrative forms to name a few. If your health care provider has added a form to your Action Plan you can complete it in advance of an appointment or at any time.

Follow these steps to complete an assigned form:

  1. Click on the Tasks bar
  2. Click on the task that is associated with the form
    • Note: If you can't find the form it is because it was scheduled you to complete on a specific date (for example week before or after). Use the left and right arrows of the Calendar to navigate to previous or future weeks to locate the form. 
  3. A new window will open with the form you are to complete. 
    • Note: If you don't see the Form, check to see if pop-ups are enabled in your browser. 
  4. Complete the form and click finish.
