Resources give you the information you need to better understand your care and take action. From self-care instructions to helpful links, this section keeps you and your support team informed—no matter where they live.
Go to the Resources block in your Action Plan
You’ll see a list of documents, websites, or videos your health team has added
Click to open, view, or download each item
🛎️ You'll get a notification when new resources are added, based on your notification preferences.
You can add documents, websites, or videos to share with your health team or your support circle.
Go to the Resources block
Click the + Add button
Choose either:
📄 Document (PDF, image, or other file from your computer/phone)
🌐 Resource (like a website or video link)
Give it a name (optional)
Click Upload
Your resource will now appear in the list and be visible to your team and supporters.
💡 Good to know
The first four resources are shown by default. Click “Show more” to see the full list.
You can upload questions or links you want feedback on (e.g., “Are these grab bars appropriate?”).
Everyone on your Action Plan—no matter where they live—can see these shared resources.
