Forms are sometimes used by your care team to collect important information—like assessments, surveys, or information gathering. Completing them helps your team understand how you're doing and tailor your care accordingly.

Why this matters

Forms help your care team gather insights without needing a visit. Whether it’s about your mood, caregiving experience, or specific symptoms, your answers help guide decisions and keep your care aligned with your needs.


How to complete a form


🔹 1. Look in your Things to Do block
If a form has been assigned, you’ll see it listed under Things to Do 

🔹 2. Click Complete now
This will open the form. Fill in the questions at your own pace.

🔹 3. Click Finish
Once you submit, your responses are sent securely to your care team.


How to review completed forms


🔹 1. Go to the Completed tab within the Things to do block
Find the form you’ve already submitted.

🔹 2. Click View Results
You can view your answers and any relevant scores, like the Caregiver Well-Being Index.

💡 You can revisit completed forms anytime—helpful for reviewing past answers or sharing with others in your care circle.

  • Some forms may be one-time, while others repeat over time
