Why this matters

Care coordination saves time, reduces effort, and helps avoid misunderstandings. Having the support of family, friends, and care providers not only improves outcomes—it also reduces stress and burnout.

Careteam helps you bring everyone together in one place, so it’s easier to communicate, share updates, and feel supported.

Your team

There are two kinds of people you can add to your Action Plan:

👥 Support team

This is your personal team—family, close friends, neighbors, caregivers, or community providers.

There are two types of supporters:

• Primary supporter
  A trusted person who can view health info, message the care team, complete tasks, and upload files (same access as you).

• Supporter
  Someone who helps out but has more limited access.

🩺 Health team

These are your healthcare providers, like your family doctor, specialists, pharmacist, or therapist.

They can view important updates and collaborate, especially during care transitions.

How to add someone

1682. Go to your Action Plan

1683. Click the Your team panel

1684. Click the ➕ Add to the team button

1685. Choose whether you're adding a support team member or a health team member

Add by email

This is the most common way to invite someone.

Steps:

• Add their name and relationship

• Choose whether to:

  • Invite them to collaborate (they’ll get an email and can join your Careteam)

  • Just save their contact info for your reference

• Choose their level of access (primary supporter or supporter)

• Enter their email address

• Click Send invite

Their status will show as pending until they accept.

Add by private access code

If you don’t have someone’s email—or don’t want to reach out by email—you can give them a private link and code instead.

This is especially helpful when:

• You're inviting a healthcare provider

• You're speaking to someone in person or virtually

Steps:

• Choose to add a health team member

• Select the custom invite link

• Copy and paste the code or link to share it directly

🔐 This code will only connect them to your Action Plan.

💡 Tips and cool things to know

• You can manage your team at any time—click their name to view or update info

• Need to resend an invite or remove someone? You can do that too

• Adding contact info without inviting them helps you keep everything in one place

• Primary supporters can act on your behalf in the platform, helping share the load
